Registration is first come, first served. Here is how it works.

  1. Register

    Fill in the registration form online or at the front desk.

  2. Your first appointment

    We go over your health history and any medications you take.

  3. We request your records

    With your permission, we ask your previous clinic to send them over.

What to bring

  • Your Ontario health card
  • A list of medications you take
  • Your previous clinic’s name and phone number

Same-day & after-hours

Need care soon?

Here is what to do, whether we are open or not.

  • Same-day appointments

    Call when we open and we will offer a same-day spot if one is free. We don’t take walk-ins.

  • Prescription renewals

    Ask your pharmacy to send us a renewal request, or book a visit if your doctor needs to see you first.

  • When we’re closed

    Call the clinic line. The message lists the after-hours options for that day.

In an emergency, call 911 or go to the nearest emergency department.
